Information and Assistance

This provides individuals with current information, including the use of assistive technology, linking individuals to the opportunities and services that are available and establish adequate follow-up procedures.

Outreach

To identify and contact isolated older persons who may have unmet service needs and to assist them in gaining access to appropriate services.

Telephone Reassurance

A telephone service to provide comfort or help to participants, staffed by volunteers. Must be on a regularly scheduled call.

Visiting

Regularly scheduled visits to the residence of older participants to provide companionship and social interaction

Education

Designed to assist individuals to acquire knowledge, skills, and experience regarding issues related to health, welfare, and well-being.

Recreation

To provide structured and unstructured activities.

(Example: cards, bingo, board games, puzzles, singing, and dancing)

Health Screening

Services that utilize diagnostic tools to test people for disease or conditions or risk factors.

Physical Fitness Programs

Providing activities for people who want to improve their strength, flexibility, endurance, muscle tone, and reflexes.

Nutrition Screenings

Effective nutrition screenings for individuals at nutrition risk to change their nutritional status.

Health Promotion

Provide educational information to consumers about preventing disease and adopting healthy lifestyles; health screening will be encouraged.
